Horizon - Away from Home Care - A Great Resource to Know About!

Pricing increases and product withdrawals have resulted in more and more customers enrolling in plans without national access and out-of-network benefits. Given this trend, it is important to understand the Horizon Away From Home Care program and the benefits that are available to certain members.

Important Notes:
-This program is not available in all states. Further, in the states that do participate, there are counties that do not. It is very important to check the location to make sure the program is available. Verification can be made by calling 973-466-8091.

-The member will be enrolled in a plan offered by the local blue cross blue shield plan and thus the benefits will not match the Horizon medical plan. The plan will have different benefits and may even require the selection of a PCP.

-The effective date of Away From Home Care Program coverage will be determined once Horizon BCBSNJ receives the completed application, and is assigned by the host BCBS plan. Please remember to sign the completed application. It may take up to 10 business days for your application to be processed.

-Once your application is processed, you will receive a letter from Horizon BCBSNJ advising you of your enrollment status in the Away From Home Care Program. If approved, you will also receive welcome materials, including a member ID card, from the Blue Cross and/or Blue Shield Plan in the state where you will be temporarily living.

-Members should make sure to read the policy documents carefully as terms and benefits can be different from their Horizon plan.

-Enrollment in the Away From Home Care Program will last no longer than one year. Generally, students receive a one year enrollment, and all other participants are limited to six months or less.

-The Away From Home Care Program is not designed for short term travelers. If a member is on vacation and needs urgent, but not immediate care, they should contact Member Services at the number on their ID card.

-Other ways to get immediate care:
-Access telemedicine through Horizon CareOnline. 24/7 access to doctor's via phone or video conference.
-Emergency Room. In the event of a true medical emergency, you should visit the nearest emergency room and you will be covered as if you are in network provided your condition was truly a medical emergency.
